What we will need from you:

• Contact information; your name, position in the organization, phone number, email, and organizations mailing address
• A detailed description about your organization and a web address, if your organization maintains one
• Most recent organizations annual report or financial overview
• The amount, or type, of proposed contribution (whether monetary or items requested)
• Time frame of when the contribution is needed by
• Proof of active 501(c)(3) status
• Description of how the donation directly impacts and/or benefits your organization and our community
